Perform POST requests with jQuery Mobile

I'd like to perform a POST request with jQuery Mobile, but I couldn't get this working.

Please have a look at https://www.vetpraxis.de/localhost where you can see an example of my problem. The grey area outputs the POST array (using print_r($_POST) in PHP), but no input fields are submitted.

I'm aware of the fact that the form tag should contain data-ajax="false" when requests should not make use of AJAX, but this didn't help.

Do you have an idea what I'm doing wrong?
